The program also provides budget counseling, goal setting, and one-on-one case management by nationally certified staff.
West Idaho Community Health Collaborative (WICHC)
Across its ten counties, the West Idaho Community Health Collaborative is making an impact. The collaboration’s mission is to improve community health by aligning healthcare, social services and public health. As a part of its efforts, the group is testing a statewide health delivery transformation model. Among other things, the ten counties will take part in community needs assessments and learn how to better communicate and coordinate care among their providers.
The West Idaho Community Health Collaborative is made up of twenty-one partners, each of which represents a subject matter expertise in a different sector. The collaboration represents half of Idaho’s population.
